The Problem

Settled in the Southwest region of West Michigan, Allegan County offers rolling farmlands, sweeping lakeshore views, and the sanctuary of sprawling forests. For those who call Allegan County home, the beauty of the land and a sense of community are what draw them in. The rural nature of Allegan County also presents a range of persistent and complex challenges for those who reside there---one of them being access to health care. Due to factors that include transportation challenges, provider shortages, lack of insurance, decentralized services, and socio-economic barriers, the need for innovative solutions for health care access continue to grow.

Our Response

At Grounded Community Health, we are focused on providing accessible, compassionate, and holistic physical and mental health care resources for those underserved and in rural communities of Allegan County.

We want to make a difference by:

Creating a model of care that provides both a central location and mobile care options for rural areas in need
Providing free physical and mental healthcare services to residents who are uninsured and under-insured
Assisting with healthcare navigation through increasingly complex healthcare systems
Providing healthcare education that focuses on preventative health strategies and lifestyle change
Incorporating interpretation services for Spanish-speaking residents
Collaborating with long-standing community organizations and businesses to streamline services and develop a coordinated network of care for residents of Allegan County
